Before You Start Your Claim

Make sure you have...

T-Mobile document

Details about your damaged or lost device.
What's the make, model, and serial number of the device you're claiming? What happened to it? When did it occur?

T-Mobile ID

Your personal info.
Have your T-Mobile PIN or user ID and password handy. You'll also be asked for contact info of the primary account holder and claimant.

T-Mobile card

Your applicable deductibles.
The service fee or deductible applicable to your claim will depend on your device and selected protection program.

T-Mobile documents

Replacement details.
For approved claims where replacement equipment will be shipped, you’ll be asked for a a shipping address (Note: P.O. Box delivery is not available). You can also get your replacement device by visiting a T-Mobile store. To locate your nearest store, click here.
