Before You Start Your Claim

Make sure you have...

Details about your damaged or lost device.
What's the make, model, and serial number of the device you're claiming? What happened to it? When did it occur?

Your personal info.
Have your myT-Mobile.com user ID and password handy, or use the last 4 digits of your social security. You'll also be asked for contact info of the primary account holder and claimant.

Once your claim is approved don't forget...

Your applicable deductible.
The service fee or deductible rate applicable to your claim will depend on your device and protection program selected.

Replacement details.
For approved claims where replacement equipment will be shipped, you’ll be asked for a a shipping address (Note: P.O. Box delivery is not available). You can also get your replacement device by visiting a T-Mobile store. To locate your nearest store, click here.
